Executive Summary

Project Description

The HEAL project is designed to:

  • Physical Activity Programs: Implement after-school and weekend sports and activity clubs, providing safe, structured environments for physical exercise.
  • Access to Healthy Foods: Partner with local farms and stores to distribute fresh produce to families and establish a community garden.

Activities will be rolled out over 12 months, starting in July 2024, with ongoing evaluation and community feedback shaping program adaptations.

Goals and Objectives

  • Reduce the rate of childhood obesity in Springfield by 10% within one year.
  • Increase participants' physical activity levels by 30% over the project duration.
  • Improve dietary habits among 80% of participating families, as measured by self-reported increases in fruit and vegetable consumption.

Evaluation Plan

Success will be evaluated using pre- and post-program surveys, body mass index (BMI) measurements, and activity logs. A third-party evaluator will conduct an impact assessment, with findings reported to stakeholders and used to refine future iterations of the project.

Budget and Funding Request

The total budget for the HEAL project is $75,000. BFCH is requesting $50,000 from the Global Health Initiatives Foundation, with the remaining $25,000 secured through local partnerships and in-kind donations. Sustainability Plan
